Register as New Patient
If you wish to register as a new patient please ask at reception for a registration form and health questionnaire or complete the online forms below.
You will be registered with the practice rather than an individual GP but you are entitled to express a preference. The practice will do its best to let you see the doctor of your choice, but it might not always be possible.
Register Your New Baby
If you are the proud parent of a new baby, you may not be thinking about the need to register him/her with a doctor, but it is in the baby’s best interest to do this as soon as you can. There are forms at the surgery that you can use to register your baby.
When the surgery is informed of your baby’s birth, we will send Mum a six week post natal appointment and your baby’s first immunisation appointment. If we do not have a registration for your baby we will include the appropriate form.
Temporary Patient Registrations
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Disabled Patient Facilities
The practice is easily accessible to wheelchair users and there is a suitable toilet available. Any patient who needs extra help can let us know at reception either in advance of the appointment or when booking in. People with a hearing difficulty can ask a receptionist to tell them when they are called. Names are shown on the screen in the waiting areas as well as a voice message when the doctor or nurse is ready to see you.
